Newfound Student and Parent COVID Vaccine Registration Form
Please see below for information about how to participate in the upcoming COVID-19 Central NH Public Health Network Vaccination Clinic. This free event is hosted by SAU 4 and is open to students aged 12 and up (with parental consent) and their families. The clinics will be held at Newfound Memorial Middle School (NMMS) starting at 8:00 am and Newfound Regional High School (NRHS) starting at 10:45 am on May 19th AND June 9th.

One registration form MUST be filled out for EACH person receiving the vaccine.

The Pfizer-Biontech COVID-19 vaccine will be administered. Information about this vaccine can be found here:

In order to be eligible to receive the vaccinations at this clinic, you must be a New Hampshire resident, must not have had any other vaccine within the 14 days before the shot, must bring valid identification or if you are a minor, you must bring a copy of the signed parent consent form, and you must review the attached questions.

For those recipients who are under 18, after we receive your registration through this form, we will send out the parent consent form. This form will be sent via email through a secure electronic signing service called PandaDoc. Parents will be able to provide consent electronically and we will have the printed copies available on the day of the clinic.

Recipients who are not students at our schools will be notified when to arrive at the site.
